CURRICULUM VITAE / RESUME :
Business CV/Resume Of Principal Applicant - Indicating academic qualification, working experiences & skills or expertise acquired.
PHOTOGRAPH :
i) Six (6) copy of passport size colored photographs of each member included in the application.
PASSPORT :
Full book (Page # 1 to Page # 48 or 64) photocopy of passport of each member included in the application (Minimum 1 year Validity).Previous passport of all applicants (If available).
MARRIAGE CERTIFICATE :
(If accompanied by spouse)Certified copy of marriage certificate of the applicant.
BIRTH CERTIFICATE :
Certified true copy of birth certificate of each child from City Corporation/Union Porisad. School Information of each child, under 21 years.
Police Clearance :
Police Clearance is compulsory for the principal applicant and the spouse and to be submitted together with the application file.
INCOME DECLARATION :
BANK STATEMENT:
- Certified copy(s) of latest 3 / 6 months Personal bank statement /FDR/Savings Certificate/Deposit Scheme/ Spouse bank statement.
- For below 50 years old of age-should have minimum balance of RM 5,00,000.00 (BDT- 1,50,00,000.00) in bank statement (Either one or combined banks) For 50 years old and above-should have minimum balance of RM 3,50,000.00 (BDT- 1,00,00,000.00).
BUSINESS DOCUMENTS:
- Business card (6 pcs).
FORWARDING LETTER:
Letter of application (cover letter) from the applicant, stating the following:
- Indicate intention to join Malaysia My Second Home Program
- With whom the applicant wishes to come (single or with family)
All compulsory photocopy documents such as photocopy passports, marriage certificate, birth certificates and pay slips must be attested by the embassy of Malaysia in Bangladesh
Note :
Accompanied children must be below 21 years old and unmarried are regarded as dependent of the applicant. Children above 21 years old must apply as an individual applicant.
